Here's how to enter...
Upon completion of a regularly scheduled cleaning and check up appointment, with an OK from the doctor that you are
"Cavity Free", we will place your name in our "No Cavity Club" jar.
On the last work day of the month we will draw a name and notify the winner by phone.
